CAPP MEMBER SURVEYS (OPTIONAL)
Thank you so much for joining the CAPP Data Registry and being a part of such an important research project. Below you will see all CAPP Data Registry Surveys available to you. If you see any that you have not taken and would like to, please feel free to log in and take them! Please DO NOT take a survey more than once.
Some new surveys may have different risks and benefits, as compared to this baseline survey, and thus may require an additional consent form or process. For every survey taken below, each participant will have one additional entry into periodic lottery drawings for a $200 cash incentive. For example, in any given month, a participant’s total number of entries into the lottery will be equal to the number of surveys they have completed at that time.
